New Patient Information

We look forward to seeing you here at Bright Valley Dental! During the first visit, we’ll make sure to obtain important background information, like your medical history, and give you time to get to know Dr. Varma.

You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment. Click here to view and print our New Patient Forms. Otherwise, please arrive 15 minutes early to your first appointment to complete the forms. If applicable, please bring your insurance card.

Things to know before booking an Appointment

Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A cancellation fee will be charged if a patient does not show up for an appointment without 24 hour notice.

Financial

We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. For patients who require major work, a complete payment plan is designed with an appropriate payment schedule. Forms of payment accepted by the office are check, cash, Visa, MasterCard, Discover, American Express and Care Credit.

No insurance? No problem! Bright Valley Dental offers a dental plan for patients without insurance. The plan includes various dental benefits such as exams, x-rays, and routine cleaning, as well as a 20% off for restorative work! Ask us about the plan during your visit.
